Have you already had a flu vaccination with Capital Health Care and did not receive a copy of your vaccination certificate? Did your MyGov health fail to update - please fill out the information below.

Please note there are two reasons your vaccination certificate did not arrive at your email address or record on the Australian Immunisation Register.
1. Incorrect email address entered at the time of appointment
2. Incorrect Medicare Card details
